Observed baked potatoes on three trays stacked on top of each other in the reach-in cooler at 60-69F at 6.5 hours. Must cool time/temperature control for safety foods from 135F to 41F within 6 hours. The potatoes were discarded. When cooling, do not stack trays on top of each other.

1. Observed a utensil in the hand wash sink, making it not accessible. The utensil was taken to be washed. Nothing can be in the hand wash sink, especially not food service equipment.



2. Employee food items stored in the walk-in cooler, interspersed with food items for service. Employee items must be stored in a container that is marked "employee food", below food for service. The items were relocated.



3. Food scoop was stored in stagnant water. The scoop must be stored in the food with handle extending out of the food, or in water held at 135F or above.



4. Clean containers were wet stacked. Must air dry before stacking. The containers were separated to air dry.

1. Observed wet wipe cloth on prep table. Wet cloths must be in sanitizing solution between uses. The cloth was placed in the sanitizer bucket.



2. Observed employee touch clothing wearing gloves. Never touch clothing or body with gloves for handling ready-to-eat food. The employee was made aware, washed hands and changed gloves.


9/22/2014 93-A

1. Observed a build-up of mold, dust and debris under the self-service beverage dispensing unit. Clean under the dispenser. Repeat Violation.



2. Observed dried food on the floor in the walk-in cooler. Clean the floor.



3. Observed flies in the kitchen area. Person in charge must ensure that the flies are controlled. Efforts were being made to reduce the amount of flies, but they were still present. Repeat Violation.

1. Clean containers wet stacked. Must air dry before stacking. Repeat Violation.



2. The shelving where clean containers are stacked not clean. Observed greasy buildup. Clean the shelving.



3. Observed numerous flies in the facility. Person in charge must ensure that the flies are controlled. Efforts were being made to reduce the numbers but they were still present.

1. Observed employee check the cook temperature of baked fish without first cleaning the probe. Use an alcohol swab to sanitize probe before checking the temperature.



2. Observed numerous flies in the facility. Person in charge must ensure that the flies are controlled.

1. Sanitizer for wipe cloths at 0 ppm quat. Must be a minimum of 200 ppm quat. New sanitizer was made.



2. Numerous food containers cracked, chipped and in disrepair. These containers must be discarded.


12/5/2012 91-A

1. Observed green beans cooling in a large container in the walk-in cooler at 68F. The beans were placed on sheet pans to rapidly bring the temperature to 41F. When cooling potentially hazardous food, always use an ice bath or shallow containers.



2. Cheese container had water condensate in it. The cheese was discarded. Place a lid on the cheese to ensure it is protected from being contaminated from the condensate.



3. Rice scoop stored with the handle in the rice. Always store the scoop with the handle extending out of the rice.



4. A back flow prevention device is needed on the hose bibb.


7/31/2012 92-A

1. Cutting board was observed to have a black build-up in the grooves from cutting. Either replace or resurface the cutting board. Another cutting board was available and was utilized. Also wet wipes should not be used for sanitizing thermometer probes. Use alcohol swabs.



2. Need thermometers for several cooling units.



3. The floor has been recently regrouted and the grout is covering the surface of the tiles. The surface should be cleaned to be smooth and easily cleanable.

1. Observed potentially hazardous food cold held at greater than 41F. Miscellaneous PHF in the prep cooler at 48F. The unit was at the maximum setting, so the unit was packed with ice until a service representative could arrive. The person-in-charge must ensure temperatures are monitored.



2. The inside of the cooling units should be cleaned to include the pooling water on the bottom shelf, the seals and shelving.

1. Observed two employees wearing watches while preparing food. The only jewelry allowed on either hands or arms is a plain band ring.



2. Wet wiping cloth not stored in sanitizing solution between uses.



3. Single service articles must be stored inverted or covered.



4. The seal on the 3 comp sink must be replaced.



5. Replace broken ceiling tile. Repeat violation.

1. Observed potentially hazardous food cold-held at greater than 41F. Miscellaneous PHF in the prep cooler at 48F. The unit was adjusted down at the time of inspection. The person-in-charge must insure temperatures are monitored.



2. Cooked potentially hazardous food not cooled from 135F to 41F within 6 hours. Green beans at 6 hours were at 51F. The green beans were discarded. Person-in-charge must insure proper cooling methods are used.



3. Clean containers wet stacked. Must air dry before stacking.



4. Replace broken ceiling tile.



5. Observed numerous flies in the facility. Person-in-charge must insure that the flies are controlled.


2/11/2011 90-A

1. Handwash sink not accessible for employee use at all times. Observed a drink and single service items in the hand wash sink. Person-in-charge must insure that the handwash sink is accessible at all times.



2. Observed green beans cooling in walk-in cooler at 96F. The containers were stored together with other items stacked on top. To properly cool there must be proper air circulation. The person-in-charge stated they had only been there 30 minutes and separated the items to insure proper cooling.



3. Observed dirty wipe cloth stored directly on top of single service articles. Person-in-charge must insure that the single service articles are protected from contamination. Corrected on-site.



4. Numerous food containers cracked, chipped and in disrepair. These containers must be discarded.



5. Observed build up on shelves in dry storage area and on cooler handles. Clean the shelves and the handles.



6. Observed numerous employee jackets and clothing stored in the dry storage area with canned and dry foods. Employee items must be stored separately, away from food and food service equipment.



7. Observed missing base tile.


8/16/2010 93-A

Equipment food-contact surfaces not sanitized at regular intervals. The surfaces should be sanitized frequently with the maximum time not to exceed 4 hours. Corrected on-site.



Numerous food containers cracked, chipped and in disrepair. These containers must be discarded.



Cold water not provided at employee handwash sink. The sink must be repaired.

Wet wiping cloth not stored in sanitizing solution between uses. Wet wiping cloths must be stored in sanitizing solution.



Scoops used for food service must have a handle and the handle must extend out of the food product.



3 compartment sink not attached and sealed to wall. The ware washing sink must be sealed to the wall to prevent moisture and growth from occurring in an area that cannot be cleaned sufficiently. Repeat violation.

Improper manual dish washing procedure. Utensils must first be rinsed before sanitizing.

Improper cooling of cooked green beans. Must rapidly cool to 70 F within 2 hours.

Utensils must be protected from splash from hand wash sink.
